About the test

Ionised calcium is the free, biologically active fraction of blood calcium that is not bound to proteins. The test helps clarify calcium balance when total calcium does not fully reflect the active fraction. It may be used in altered albumin levels, impaired kidney function and disorders of mineral metabolism regulation.

Ionised calcium depends on pH and sample handling conditions. It cannot be directly compared with total calcium as though they were the same measurement. The doctor assesses the result together with symptoms, medical history and other investigations. The reference values of the individual laboratory are used for comparison, and changes over time are considered during follow-up.

Why it is ordered

  • Assess the active calcium fraction.
  • Clarify disorders of calcium metabolism.

How to prepare

Before taking the test, check the preparation and specimen collection requirements with the laboratory. Tell your doctor about medicines and recent treatment. Do not stop prescribed medicines on your own.
